    Building Use Agreement(Required)
    1. User agrees that it will not use the premises for any unlawful purposes, and will obey all laws, rules, and regulations of all governmental authorities as well as agreed upon policies and requirements of PTCUMC attached to this agreement.

    2. If deemed necessary by a church representative, User promises and warrants that it carries liability insurance with a minimum liability occurrence limit of $1,000,000. The User will provide a certificate of insurance (COI) to PTCUMC at least 14 days prior to the date upon which the User begins to use the above described premises. The COI will indicate that User has made PTCUMC an “additional insured” on User’s policy with respect to the use by User of the above described premises. The COI must be sent directly from the insurance company to PTCUMC by using FAX #770-487-0203.

    3. User agrees to conduct a visual inspection of the premises, including entrances and exits, prior to use, and warrants that the premises will be used only if it is in a safe condition.

    4. User agrees to hold harmless, indemnify and defend PTCUMC (including PTCUMC’s agents, employees, and members) from any and all liability for injury or damage which may result from any person using the above described premises, its entrances and exits, and surrounding areas, for User’s purposes, regardless of whether such injury or damage results from the negligence of PTCUMC (including PTCUMC’s agents, employees, and members) or otherwise.

    5. User agrees to be responsible for preparing for use and returning to the pre-use condition all areas of the premises which User will use, including entrances and exits. If a security deposit has been required, failure to return facility to pre-use condition may result in forfeiture of security deposit.

    6. Should User request that PTCUMC staff set up and take down in preparation of the event, there will be a charge of $150.

    7. This agreement may be cancelled unilaterally by either party with a 60-day written notice. 100% of the room fee is refundable if a 60-day written notice is given. Should the notice be from 59 to 30 days, 50% of the room fee is refundable. If notice is given less than 30 days until the event date, the room fee is not refunded. If PTCUMC is the party that cancels use of the room at any point, the room use fee is refunded in full. We reserve the right to cancel, change or amend the use of facilities at any time, in order to meet the programming needs of the Church. We will provide, if possible, seventy-two hours’ notice of any potential changes or amendments to reservations for specific rooms or areas, and thirty days’ notice, should cancellation of an entire event or ongoing reservation become necessary.

    8. User agrees that it will not assign any of its rights under this agreement, and any such assignment will void this agreement at the sole option of PTCUMC.

    9. PTCUMC and User agree that any disputes arising under this agreement will be resolved via a mutually acceptable alternative dispute process. If PTCUMC and User cannot mutually agree upon such a process, the dispute will be submitted to a three-member arbitration panel of the American Arbitration Association for final resolution.

    10. This document contains the entire agreement of the parties and supersedes all prior written or oral agreements relating to the subject matter.

    11. Per our Safe Sanctuaries Policy, please note that any event scheduled from 9:00am – 1:00pm Monday-Friday does not have access to any areas on the 100 level. If you need to access that area for any reason, please ask a staff member to escort you.
    Waiver of Liability(Required)
    In consideration of the use of the building facilities and/or the property of Peachtree City United Methodist Church, the undersigned organization on behalf of the organization and any of its members and any persons under its direction and control, hereby waives any claims, demands, or causes of action which may arise or be deemed to have arisen from any injury, at such facilities used by the organizations, its members or persons under its direction and control, and the said organization hereby agrees to hold harmless Peachtree City United Methodist Church from any such claims, demands or causes of action.

    Should the organization use the facilities and/or property of Peachtree City United Methodist Church on more than one occasion, this waiver shall be deemed a continuing waiver effective for each such use by the organization, its members or persons using the facilities under its direction and control.
    Safe Sanctuaries(Required)
    I acknowledge that I will ensure my group's (or activity's) compliance with the Safe Sanctuaries Policy of Peachtree City United Methodist Church for Reducing the Risk of Child and Youth Physical and Sexual Abuse.

    All church groups and outside groups that sponsor events involving children or youth under the age of 18 years old must abide by the Safe Sanctuary Policy. When children or youth are in attendance, they must be under the control of their parents or adults at all times and are not permitted to roam freely on church property. No youth or children may be left alone with 1 adult unless said adult is parent of all children present. There must be 2 adults (other than husband and wife) present with children and youth at all times unless said adults are parents of all children or youth present.
